The client intake form is intended to be completed by each waxing client to gather important information ahead of their waxing service. The form should be filled out by all new clients, but it is good practice to have current clients check their intake form each visit to ensure all information is to up to date.

GETTING IN TOUCH
For convenience, the client intake form can be sent prior to the service via e-mail, or a link to the form can be sent via text. This will save time and allow the waxer to gather information ahead of the service and be adequately prepared. In some instances, there may be a need to contact the client ahead of the service, as there are certain medications or ingredients in skin care products that can adversely affect their wax. For example, it is best to refrain from using retinol or glycolic acid 48 to 72 hours prior to a facial wax. Whichever way the waxer decides to administer the form, they will want to make sure that it is easy to complete and does not take too much time.
INTAKE FORM CONTENTS
When creating the form, consider what information is needed to ensure the safety of clients and minimize any adverse reactions. Questions should include what medications a client is currently taking. This is important, as some medications such as isotretinoin can affect skin’s sensitivity and may cause adverse reactions to waxing. It is also important to ask if a client has been tanning or intends to tan within the next 24 hours. It is advisable to avoid tanning 24 to 48 hours after waxing because it can negatively impact skin. As all skin reacts differently with waxing, it is good practice to discuss the possible side effects of waxing with clients so that they are aware.
If a spa offers male waxing, specifically male intimate waxing, it is a good idea to include a disclosure on the form that indicates the possibility of situations in the wax room and the level of professionalism the spa adheres too. The form could also indicate the confidentiality between the waxer and their clients. Being upfront and open with these situations may help to avoid any awkward situations as well as protect the spa and service provider.
Before qualifying clients for waxing services, service providers should request comprehensive background information through their intake form and regularly check in with existing clientele.. This will provide a safer and more successful experience for both clients and the spa.
